Many reasons may exist for this, namely that it is difficult to get a higher concentration of medical experts to live and work and sign long-term commitments to live and work in rural settings which are far-removed from urban areas with higher concetrations and closer proximity to necessary other specialists and machines.
It seems to be easier to bring the rural patients to the urban medical care facilities, than to bring the continuum of specialists to the rural areas.
